Write your claims on daysheets and we will input them for you. You may photocopy our standard format daysheets, download a Type 1 or Type 2 daysheet from herer, or use your own forms provided they are conducive to our data entry format. They may be faxed to us, sent by mail or courier or by encrypted e-mail over the Internet. Your daysheets and reports will be returned to you the week following submission at no additional cost.
